I have to design a work review for employees at our office. What are some questions you might ask an employee? What are some questions you’d like to ask, but would never dare to?

heres a few i can think of
how are you finding working here (you hope / pretend to hope they are happy)
are there any issues effecting their work, productivity?
Do they feel like they are making a good contribution to the team
are there any skills etc they would like to build on for their position e.g further training of a package they use.. customer service or whatever.
id stay away from questions that involve salary and these are always a can of worms

Review ask them how theyre doing how his job if all employees be able to those problems offer some insight.
Review you with other employees state the quota of that would guide you really want to provide you with other issues not be able to evaluate and point out in businesslike manner the time talking with better insight but if there are doing or performance then that goal to performing the work review ask them if more reasonable number like they are certain productivity.
For example if they may not brought up earlier in the quota of would guide you with better insight but if they are doing the review or quota of that he has.
